General Information

Every student-athlete that participates in a Bixby school-sponsored sport(s) must have an annual Physical (after May 1st) on file before any physical participation is allowed.

Student-athlete or guardian must turn in a completed physical form (CLICK HERE FOR PHYSICAL FORM) and parent consent form (CLICK HERE FOR PARENT CONSENT FORM) to the Athletic Department or to the Certified Athletic Trainer to complete all eligibility requirements.
